Accepting Applications for the 2013 Partnership in Ministry Grants

Ascension Health is accepting applications for the 2013 Partnership in Ministry Grant. Grants are awarded for up to two years and in amounts up to $50,000 per year. Grants support an ongoing, established program or service or a start-up program. Seton Institute is Ascension health’s primary support for international programs. A small number of international grants up to $10,000 may be given funding.

Eligibility

The Ascension Health Partnership in Ministry Grant was established to recognize and promote integration and collaboration among the social, education, parish and healthcare ministries of the Daughters of Charity, the Congregation of Saint Joseph, the Sisters of Saint Joseph of Carondelet and Alexian Brothers.

An application may be submitted by any Health Ministry or service organization formally associated with one of the groups listed above. “Formally associated” means that a member of one of these groups is employed in or directly responsible for the organization or nominated program.

A program receiving a major grant will not be eligible to reapply within four years of receiving that grant.

Entry Process
Entrants should submit a summary of how their project or service has or will make a difference using the listed criteria. Each one of the criteria must be addressed for the grant to be considered. Entries should be submitted on this entry form.

Given the large number of applications we receive, we encourage applicants to select the one or two proposals in each category that best suit the purpose of this grant.

Entry Deadline
Applications for the 2013 Ascension Health Partnership in Ministry Grant must be received no later than April 19, 2013.

Selection Process
Applications will be reviewed by the Ascension Health Partnership in Ministry Task Force. Recipients will be contacted and the Ascension Health Partnership in Ministry Grant will be awarded in July 2013. To assure the best possible consideration of your application, please follow all instructions carefully.

Criteria
Following are the criteria used in the selection process for the Ascension Health Partnership in Ministry Grant. Each program or service will be evaluated (see questions 4 and 5) to ensure that it:

Encompasses the Mission, Vision and Values of Ascension Health

  • Advances the health within the local community
  • Serves those most in need – the unserved/underserved
  • Promotes social justice

Promotes Integration and Networking

  • Promotes collaboration
  • Demonstrates networking; e.g. with social, education, parish or other healthcare ministries

Offers New Dimension or Advancement in Care or Service

  • Promotes education and prevention
  • Supports a holistic model of care (body, mind and spirit)
  • Uses Resources Responsibly

Promotes good stewardship of resources

  • Exemplifies a cost-effective initiative
  • Delivers expected outcomes
  • Engages the broader community in meeting this need where possible – demonstrates commitment to partnerships
  • Demonstrates the potential for ongoing sustainability (The Ascension Health Partnership in Ministry Grant is not intended to subsidize already-funded salaries and/or operating expenses.)

The 2012 Partnership in Ministry Grants

The Ascension Health Partnership in Ministry grant program was established to recognize and promote integration and collaboration among the social, educational, parish and healthcare ministries of the organization’s Sponsors. Any Health Ministry or organization directly associated with the Daughters of Charity, the Congregation of St. Joseph or the Sisters of St. Joseph of Carondelet may submit an application for grants. Selected programs or services must meet four criteria: encompass the Mission, Vision and Values of Ascension Health; promote integration and networking; offer new dimensions or advancement in care or service; and, use resources responsibly.
